Presidents Choice Chocolate Milkshakes how to make or where to buy?

Alright, this apparently has been discontinued. I am going to have this milkshake no matter what it takes. PC told me they wont give me the name of the manufacturer that makes this, and I cant find it. Does anyone have any ideas on where to find this information? if not, does anyone know how to take ingredients from the milkshake and make it yourself? My problem is I dont know where to get alot of the ingredients, how they are combined, or how much of each ingredient to put into it. Any help in getting this item at all will be life saving to me. This milkshake has been around for at least 15 yrs, i used to drink it when i was little, and just recently has been discontinued. Thanks for any direction anyone can give me on this.
